We have learned to look after the body. We read the labels. We count the steps. We know what a good night’s sleep is worth and roughly how much water we are meant to drink. Wellness has become a project we manage with real discipline: eat the plants, move the muscles, lift the weights, log the runs.
And yet there is one practice we leave out of the plan almost entirely. The practice of doing nothing. The practice of stillness.
We treat breath-work and meditation as something for the mind, if we think of them at all, a nice-to-have for a stressful week, a spa-day flourish, a bit of self-care. But stress does not stay politely in the head. It moves into the body. It settles in the shoulders and the jaw, in the gut, in the blood pressure reading, in the way you catch every cold going round. The evidence for this is not soft. And the evidence that stillness can undo some of it is not soft either.
This is the case for putting breathwork and meditation on the same shelf as diet and exercise. Not as a luxury. As maintenance.
Stress isn’t only in your head. It lives in your body.
When you feel threatened by a deadline, a difficult email, a long enough stretch of low-grade pressure your body releases cortisol and adrenaline and moves into fight-or-flight. In short bursts this is exactly what it should do. The problem is modern life rarely lets the alarm switch off. The response that was built for a genuine emergency ends up running quietly in the background for years.
Clinicians are clear about where that leads. Chronic stress has effects on almost every system in the body, from the heart to the digestive system to the immune system. Prolonged cortisol keeps blood pressure elevated, which raises the long-term risk of heart disease. It suppresses the immune system by reducing your supply of white blood cells, which is why a hard project so often ends in a cold. It disturbs digestion and is linked to conditions like IBS and acid reflux. It tightens the muscles, feeds tension headaches, and it keeps the body flooded with the inflammation that leaves us vulnerable to chronic disease.
The most striking finding sits at the level of the cell. In 2004, health psychologist Elissa Epel and Nobel laureate Elizabeth Blackburn published a study in PNAS looking at mothers under sustained caregiving stress. They measured telomeres, the protective caps on the ends of our chromosomes that shorten as we age. The women under the most chronic stress had telomeres so much shorter that it corresponded to roughly a decade or more of additional biological ageing compared with less-stressed peers. Stress, it turned out, can age you from the inside out.
So when we say breathwork and meditation are “good for stress,” this is what is actually at stake. Not a mood. A body.
The pillar of wellbeing we quietly skip
Ask most people to name the foundations of good health and they will get to diet, exercise and sleep quickly. All three are, rightly, mainstream. There is a fourth that belongs beside them and almost never makes the list: the regulation of the nervous system. The deliberate practice of coming out of fight-or-flight and back into what physiologists call “rest-and-digest.”
The irony is that the tools we reach for to feel well can quietly pull us the other way. The fitness tracker that pings all day, the ring that scores your sleep and then makes you anxious about the score, the wrist that buzzes with someone else’s urgency. So much of the technology sold to us in the name of health is built to capture attention. Stillness asks for the opposite. It asks for your attention back.
Here is the reassuring part. The body responds to stillness quickly, and the research is now specific about how.
What breathwork does to the body
Breathing is the one branch of the autonomic nervous system you can take hold of directly. You cannot consciously lower your heart rate or widen a blood vessel, but you can change your breath and through the breath, reach the rest.
It lowers cortisol. A 2025 narrative review of thirty breathwork studies found a consistent pattern: slow, nasal, diaphragmatic breathing improves vagal tone and reduces cortisol, anxiety and perceived stress. In controlled trials, participants doing structured breathing have shown measurably lower salivary cortisol than control groups who did nothing, a physical drop in the body’s main stress hormone, not just a reported sense of calm.
It shifts you out of fight-or-flight. Across that same body of research, heart rate variability (HRV) a well-regarded marker of cardiovascular health and nervous-system flexibility was the single most improved measure, rising in the large majority of studies. Higher HRV reflects a nervous system that can move fluidly between effort and recovery rather than being stuck on high alert. Slow breathing is one of the most reliable ways to raise it.
It can lower blood pressure. Slow, paced breathing has enough of a track record here that a device delivering it (RESPeRATE) has been cleared by the US Food and Drug Administration for stress and blood-pressure reduction. In a twelve-week trial at Vanderbilt University, healthy adults practising slow breathing saw reductions in blood pressure. The effects reported across the literature are typically modest, but they are real and they compound.
It works through the exhale. The mechanism is elegant. Slowing the breath especially lengthening the out-breath activates pulmonary stretch receptors and the vagus nerve, engages the baroreflex that governs blood pressure, and turns down the sympathetic “fight-or-flight” signal. This is why the breathing patterns that research favours share a shape: a slow rate of around six breaths per minute, and an exhale at least as long as the inhale. It is precisely the shape of the patterns people already use by name, box breathing (in for four, hold for four, out for four, hold for four) and the 4-7-8 (in for four, hold for seven, out for eight). These are not folk remedies. They are the applied physiology of the exhale.
What meditation does to the body
Where breathwork is the fast route in, meditation is the long game and its benefits show up in some of the body’s deepest systems.
It calms inflammation. Inflammation is the quiet thread running through so much chronic disease, and meditation appears to lower it. In a randomised controlled trial, older adults practising mindfulness showed significantly reduced high-sensitivity C-reactive protein, a key marker of inflammation, at nine months. In another randomised trial, patients living with end-stage renal disease saw significant reductions in both C-reactive protein and tumour necrosis factor after a mindfulness meditation programme. These are measurable changes in the blood, not impressions on a questionnaire.
It supports the heart. In 2017 the American Heart Association reviewed the evidence and issued a scientific statement concluding that meditation may reasonably be considered as an adjunct to established, guideline-directed ways of reducing cardiovascular risk alongside, not instead of, the usual care. The AHA was careful, and so are we: it framed the benefit as promising rather than proven, and called for more rigorous trials. But for a practice that is free, low-risk and available to anyone, “may help the heart, and is very unlikely to hurt it” is a strong place to stand.
It may slow the ageing of your cells. Remember those telomeres that chronic stress shortens? The research that began with Epel and Blackburn has since asked the reverse question whether stillness protects them. Meta-analyses of mindfulness interventions find a small but consistent positive effect on telomere length and on telomerase, the enzyme that helps rebuild telomeres. The science here is still early and should be held lightly. But the direction is quietly hopeful: the same lever that stress pulls one way, practice may nudge back the other.
It helps the whole system settle. Slow-paced breathing and meditation have also been linked in review work to activation of anti-inflammatory pathways, improved lung capacity, and better sleep quality, the physiological ground on which everything else, including recovery from exercise, is built.
Stillness is a practice, not a mood
None of this asks for an hour on a cushion or a retreat in the hills. The studies above are built on minutes, done regularly. A few slow breaths before a meeting. A short sitting in the morning and the evening. The body does not require grand gestures. It requires repetition, the same way a muscle does.
We think this is the missing pillar. You would not expect to eat well once and be nourished for a year, or run once and call yourself fit. Stillness is the same. It is not an event you attend. It is a practice you keep.

How long until I feel the benefits?
Some effects, a slower heart rate, a sense of settling arrive within a single session. The deeper physiological changes seen in the studies, from inflammation to blood pressure, tend to build over weeks of regular practice, which is why consistency matters more than duration.
Is stillness really as important as diet and exercise?
Given what chronic stress does to the heart, the immune system, digestion and even the ageing of our cells, there’s a strong case that regulating the nervous system deserves a place beside the other pillars of health not above them, but no longer left off the list.
